Lortab 5 is a combination of 5mg hydrocodone and 500mg acetaminophen so both contain acetaminophen and an opioid.  Tylenol 4 with codeine is for mild to moderate pain and Lortab is for moderate to severe pain, so Lortab would probably be a little stronger.

Can vicodin show up in a urine test as codeine?

I can't say for sure, but from what I've been able to find, codeine can sometimes show up as hydrocodone (Vicodin) but not the other way around.
