Q: Nerve pain from lower back AND neck (radiating)

Hi. I'm Barb. I got into a dirtbike accident July of 2007. I decided to hop on a 450 (2 wheeler), got the throttle stuck, and ran into a 5' wide tree @ about 30 mph. Luckily, I was enough to the right (and had my helmet on), that it struck between my neck and shoulder, which made me do the end-over-end about 6-7 times, then skidded along the dirt for about 40' on my stomach with my legs UP OVER my head (kind of folded in half). I was DUMB, for lack of other words, and didn't go to the emergency room until a few months later. By then most of the bruising had vanished (3/4 of my body was BLACK) and the Dr. told me that he saw nothing on my neck xray, and I'm just sore. The second time I went, I explained AGAIN what had happened, the Dr. made me sit on the bed and place my knees together. He then grabbed them and wiggeled them back and forth and asked me if that hurt, and I said NO. He said there is nothing more he can do. That was the extent of my medical visits until recently. I finally got a referral to an Orthopedic Surgeon to look at me. It kind of sucks because I'm on Oregon Health Plan (OHP) and there just isnt much that they will pay for when you really need it! So I got an MRI on my right shoulder and left knee (seems my kneecaps are in the wrong spot, and they have to be put where they go), and am awaiting the results of those here in the next couple days. Now, my lower back and neck are causing me SO much pain, that I cry myself to sleep at night. =( My left hip goes "out" a lot, then I have sharp pains all down my left leg that makes it difficult to walk at times, and it goes numb. Also, it twitches a lot, is that normal? When it comes to my neck, it makes a really LOUD crack sound when I can get it to. When it needs to be popped, I get HORRIBLE migraines, and can hardly turn my head. I also am having the issues of lack of concentration (BAD), my speech is affected to where I can hear in my head what I am trying to say, but it wont come out of my mouth! Or if it does, it's all backwards, or I say UH a lot between words. With that, it is worse more times than others. I also cannot hardly drive anymore because my vision is all blury!! I am running into walls and door jams, and tripping over my own 2 feet A LOT.  I am getting pretty depressed, and am SOOOO Tired all of the time! I have to take stimulants like caffeine and tea and synepherine supplements to make it through the day, then I wear my self out that way....The stupid insurance company keeps denying my pre-approval for a neck and back MRI saying there arent signs of nerve damage...phooey if there arent signs! So, this seems to be more of a rant post than question post, heh. My QESTIONS are 1) does ANYone have any words of advice or resources for me to look into? 2) Is there anyone out there with the same issues that Im having? 3) What are some alternatives to medicines (other than percocet, vicodin, norco, flexeril) that can help better, but not make me lethargic?? I am currently in the process of getting my "Green Card", as that seems to be the only thing helping with the muscle spasms, migraines and nausea so far....

Wow. This is way beyond my 'pay grade' but I just wanted to wish you good luck. I hope that despite all the fatigue and pain and the rejection from the insurance companies and the less than helpful doctors that you'll keep pushing until you find a doctor you can trust and can get some answers. 

 

You might keep fibromyalgia in mind if the tests don't end up showing something that can really explain your pain and your symptoms. FM is often initiated by an accident or infection and usually shows up as body pain that spreads over time and is often coupled with severe fatigue, sleep problems and 'fibro-fog' - the inability to concentrate, a tendency to garble your words, etc, sensitivity to odors, touch, etc. Basically in FM the a part of the central nervous system appears to go haywire.

Incredibly you are alive, walking, and talking. Even though you had your helmet on, I would bet you had a closed head injury (CHI). There are various degrees of CHI's, but even the mild ones can lead to Post Concussive Syndrome. The symptoms of post concussive syndrome include: nausea, headache, dizziness, confusion, poor concentration, speech difficulty (like trouble finding words). Unfortunately, you are a year past your injury. That time point is critical because the first year is when we see the maximum improvement after a head injury. Not to say that things won't ever improve after a year, but the changes come very gradually with some good rehabilitation. Now, your neck injury probably represents ligamentous and joint damage. Cervical injuries can also lead to headaches, called cervicogenic headaches. You might do well to find a therapist who can teach you self tractioning and mobilization techniques (maybe even a chiropractor who specializes in the upper cervical spine).

 

Down to your leg, the left leg which has numbness and tingling most likely is effected by some type of nerve irritation, possible a herniated disc. If you cannot get an MRI, an electrodiagnostic test (nerve test) can help a great deal in determining how much damage is done.

 

Overall, a good neurologist or physical medicine (PM&R) doctor should be able to help you sort the injuries out and identify anything that can be fixed.

 

Medication that may be useful include anti-convulsants (used for seizures) because they soothe the injuried nerves. But a word of warning: Assuming you had a head injury, most people with head injuries become very sensitive to medication side effects like drowsiness. Provigil can help with drowsiness.

 

Finally, you will need to be extra cautious from here on out. Injuries, especially head injuries, tend to accumulate. A second, even minor injury, could greatly compound your problems.

I agree with the Dr. about CHI.  I have CHI actually TBI from a fall (11ft to concrete floor) 4 head bleeds, plus numerous fractures and breaks.  All healed now but I suffer alot, from the residual stuff.  I got with a GREAT neurologist and she put me on Aricept.  I found great success with this. Helped with forget ful ness, finding those lost words, and kinda calm my anger outbursts.  We are looking into finding somjething else for Aricept (best taken at nite) has lots of side affects. It is a long haul and brain exercises do help. AARP actually has some good onees.  I know the radiating pain down your arm from your neclk. PT could help with that, exercises correct pillow, correct posture, ect..  Just some thoughts, I pray you are well.  MK
