My husband and I both are experiecing the same problem. At first it would come and go infrequently, now his is much more frequent and stays longer. The pain is so bad in his it makes it hard for him to walk on them. The intense burning has gone away in mine, but am left with a low grade permanent stinging sensation, and if I touch the top of my big toe it feels like raw nerve. I also have a bulge on top of the same foot, it's soft tissue but definately a swollen area. Been there for about a year. Neither one of us have had foot injury or surgeries. Been to two doctors, neither of which diagnosed it. One gave me very expensive foot inserts for my flat fleet believing that was my  problem, which at 50 had never caused anything like that before so doubtful that was the problem.
