Those who have violated contracts, who have lied, cheated, and stolen in order to get medication do not have to be detox'ed by the pain clinic. The person can simply be told about the nearest addiction treatment clinic and cut loose to fend for themselves. It is actually illegal for doctors to treat addiction in those with known addiction without a special license or certification. Only certain clinics and doctors can detox an addict. There is a moral obligation and duty to help one another, but don't hold your breath.

It's all about the money.  If you have some you can get help. If you have none, unless

you live in San Francisco, you are likely on your own.  It is likely there is also some legal obligation, but there is no enforcement mechanism (again) unless you have $$$$$ for the legal team to beat up the doctor who got you hooked.  Good luck, scotto
