How does a screw in my back become fractured?

Hello;I am 46 yrs old.I have had two back surgeries in 1992 and 1994.Well it did not work so good for me.I have a fractured screw,and pain all the time.I almost died after one of my surgeries,so needless to say im scared to have one again,to fix that darn screw.The Doctor never told me how this can...
