The symptoms can be quite similar.  The hallmark of metatarsalgia is pain in the ball of the foot during weight-bearing activities (running, walking, standing, etc.). Sharp or shooting pain in the toes, and pain in the toes and/or ball of the foot that increases when the toes are flexed may also be present and can help distinguish metatarsalgia.  Some patients describe feeling like they are walking on pebbles.

 

With peripheral neuropathy of the foot, the predominant symptoms are pain, numbness, tingling, burning or a loss of feeling.  Sometimes the skin can become so sensitive that the slightest touch is painful; or the opposite may occur – numbness and a complete lack of feeling.  Some patients describe feeling like they are wearing a sock when they are not.
