Q: Severe pain

I am a 71 year old male with a long history of asthma/copd.  I am under the care of a pulmonologist and my symptoms are well managed with appropriate medication.  Recently I experienced extremely severe chronic pain in the upper right quadrant of my chest and back.   An ER doctor examined me and diagnosed the condition as costochronditis.  The pain has not responded to non-steroidal anti-inflamatory or moderate doses of steroidal medications. Despite my age, I have a full-time job that requires high levels of mental concentration.  The pain I am experiencing is disrupting my sleep and making it extremely difficult to do my job.  What other options are available to bring this pain under control, besides the medications that have already been tried?
