Q: How do you distinguish between ADHD and Bi-polar in a 9 year old child?

I am a mom with a child who has been diagnoised with ADHD with ODD. But everything I have read and also have spoken to my own doctor (not my child's) states that he should have been diagnoised with bi-polar disorder with the family history involved. I was diagnoised 8 years ago with bi-polar and my own father is also bi-polar. I have tried to explain this to my son's doctor and he just states that this is what he feels and he does not want to rule out the possiblity of the ADHD first. My son gets very angry at school and at home but does not express with words. He expresses through actions more. His doctor also has placed him on all the prescriptions that goes with ADHD and nothing helps. I could really use the help. My son's father and myself have been divorced for 5 years now. Unfortunately, my son lives with his father most of the time we share our son every other week. I see most of the episodes right before he has to go back to his fathers' but, I hear about the things that happen at school also. I am not sure what his father does when he has him but, I pay alot of attention to him and when he has outbursts with me we talk through them. I am not sure how to punish him with this but, I know his father per the child leaves him in his room for days after the episode only to be able to get out for dinner and to go to school. This worries me more. I really good use some good help. I am just worried about his well being and mental state. Should I take my child to another doctor or should I let this doctor keep doing what he is doing and hopefully my child gets better in time?

Hi, Tracy.  I'm sorry your son is going through all this.  I do think it wouldn't hurt to get another opinion.  Is your son's doctor a specialist?  If not, for sure I would get the opinion of one.  The thing that bothers me, though, is hearing how his father keeps him in his room for days at a time as punishment.  That could be causing behavior problems right there, as it seems rather extreme to me.  He may not have the best control over his behavior and that isn't his fault.  If you don't feel comfortable with what's being done for him, by all means, seek a second opinion.  Has he been evaluated by a psychologist?

I have a 32-year-old son who is developmentally disabled and autistic and he does have attention problems, for which he takes Concerta.  He's also on an antidepressant and Abilify.  Without them, he can become very irritable and angry and even suicidal.  He's never been diagnosed with bipolar, although I think at some point one doctor might have suspected it and put him on Lithium, from which he suffered bad side effects.  Anyway, it may not be the same kind of thing, but whether or not he's bipolar, whatever he's taking isn't working.  Unfortunately, it can sometimes take a lot of trial and error to get it right.  Follow your instincts - I've always found you're better off for it.  At least you can get all the bases covered and feel like you've done everything you could.

I hope you'll let us know what happens with him; sharing our stories can really help others, even if we don't know it.  I wish you all the best.
