I'm not sure about Zoloft decreasing the effect of the Ultram, but you need to be very careful about combining Ultram with any antidepressant, as there is a risk of seizures.

IM no doctor by anymeans but zoloft shouldnt reduce tramadols effectiveness it is more likely that your body is becoming accustom to the Tramadol. Try this it helps alot as I to have chronic pain. Take two max strength Tylenol (500MG) with the tramadol it helps and make it more effective. You can ask any doctor and they should agree with that. They don't know why it helps the effectiveness of Tramadol they just know that it Does.

Zoloft is a seratonin reuptake inhibitor and as such has the potential to cause seizures when given with tramadol. In addition, the mechanism of action of each of these drugs would cause one to preditct that the effectiveness of tramadol when given with zoloft will be reduced. There is a warning that these two drugs should not normaly be given together and if they are given together, the patient should be closely monitored for signs of seratonin syndrome or increased use of tramadol to control pain.

I can't tell you the exact mechanism, but Zoloft is considered an Seratonin Reuptake Inhibitor.  Criously, Ultram does the same thing.  It's a dangerous mixture that I was not aware of 4 years that lead to a seizure (seratonin syndrome).  I would not advise mixing the two drugs, or Ultram with any SSRI Class Drug.  The risks are too great.  For more infomation, Google zoloft and ultram.  Or go to http://www.drugs.com/drug-interactions/ultram-with-zoloft-2221-1474-2057-1348.html.  I hope this helps although it is a bit complicated
