It sounds like it's the pain that's preventing you from sleeping?? If the pain medication you're taking now doesn't give you enough relief, the first thing would be to try an alternative. If it's not the pain - some people take anti-anxiety medication before turning in to help calm their nerves overall.
I no longer take any sort of sleeping pill since the medical advice I have is that they interfere with your body's efforts to restore its normal rhythm.
As far as chronic pain management goes, there are other approaches to try if medication isn't satisfactory. Acupuncture works for many people - and there is an approach emphasizing mindfulness meditation outlined in Jon Kabat-Zinn's Full Catastrophe Living.

I can't help much with a direct answer, but you may want to ask this on the Chronic Pain site, too. There is also a chronic pain forum where people discuss these issues, like medications. I hope this helps a little.
