Is this part of an assigment or your internship to find these screening instruments on your own? I would start by asking your supervisors.
There are a number of screening assessments you can find on-line but whether or not they are reliable is questionable. A psychiatrist or psychologist would be the folk to be able accurately diagnose someone for these disorders.
We do have one "Depression Test: Is it Depression or Something Else?" you can take a look at. Again I would caution you that this sort of quiz is not for diagnostic purposes.
And here is an study which evaluates Breslau's 7-item screen for posttraumatic stress disorder (PTSD) for use in primary care.

It isn't an assignment; I was researching on my own initiative. I did ask my supervisor and she told me I could ask questions during the assessment to determine if client had depression. The program manager at the agency told me she wanted me to start screening for those disorders. They don't have any questions/forms or any type of screening instrument. I wanted something short and easy to understand because the veterans' reading comprehension skills aren't that high. Also, I wanted a paper/pencil and on line assessment.
