Q: Another Lantus question!

I'm still trying to get used to my new regime of Lantus and Novarapid after many years only taking two injections daily.I take a my Lantus dose of 18units everyday at 6pm and usually eat dinner around half six with my novarapid dose. Because Lantus is peakless and supposed to keep your blood sugar consistant, will my blood sugar at 6pm have an effect on my overall readings for the 23 and a bit hours that it works?.

 

For example if my Blood Sugar was high at 6pm and I take my Lantus as usual will I have a consistant high background Blood Sugar until 6pm the next day?Undecided

Hi Dee,

 

This is a tricky question.

 

If you're high at 6pm, it could be because of a meal you ate earlier and you didn't get enough novorapid insulin for.

 

If you're high at 6pm every single day, maybe you need to ask your doctor about increasing your Lantus dose.

 

You're right that Lantus doesn't peak, so if you're finding that your novorapid dose doesn't cover your carbs the way it should, you're either not getting enough coverage for your carbs and in general you need more Lantus insulin.You don't want to wait for your Lantus dose to bring your 6pm blood sugar down though, you'd want to take a bolus of novorapid insulin, using your correction factor (the amount of insulin it takes to bring you down to 120. 1 unit for me, brings me down about 50 points).

 

18 units certainly isn't very much, so it wouldn't be surprising if you needed more. I take 18 units but I am very very active every day and therefore much more sensitive to insulin than I was two years ago, when I took 35 units of Lantus.

 

Talk to your doctor about increasing your dose just a couple units at a time at first. She may suggest you try taking 20 units for a few days, then increase to 22 if you're still finding yourself high too often.

That's great thanks for your help. I actually started off on 20 units of Lantus but was waking up low every morning so my doctor decreased my dose to 18 units. This is working well for me, I just had the question, as last week I was quite high before taking my Lantus due to too much for lunch and not enough Novarapid,and wondered whether taking Lantus when bs is high determined the background being high until 6pm the next day.
