Had my first patient today that discontinued his Avandia after speaking to his MD last month but increased his Lantus and still takes glyburide.  A1c is at 6.8%.  He's doing well except for the additional 30 pounds he gained while on the medication for about 18 months.  That's what I have seen my patients on "paper" look good with the lower A1c but most have gained weight.  Some have come to my office and while in the dental chair I look down at their feet and see their ankles quite swollen.  When asked if their doctor was concerned most of the time they were not concerned.
