Actually the UKPDS showed that the 7% A1c isn't anywhere near as healthy for Type 2s as it is for Type 1s, because Type 2s achieve that 7% A1c by being high all the time, where Type 1s oscillate very high and then very low. The prolonged exposure to high blood sugars seems to be what does the microvascular damage.

  

If Type 2s could shoot for a lower, healthier A1c--mid 5%, for example,, by keeping their blood sugars under the AACE target of 140 mg/dl, they would almost certainly prevent complications.  

 

And if they got some training in how to use bolus insulin, which very few do, rather than just using basal insulin which has no impact on the very high carb meals they are encouraged to eat, they could avoid complications too.


But this takes educating and doctors don't seem to understand that Type 2s need diabetes education just as much as Type 1s. And Basal/Bolus regimens, not just Lantus.
