Changing the Timing of Lantus Injection - Makes Quite a Difference!

  • Isn’t it funny when a mistake leads you to discover something beneficial?  I had this happen to me last week and thought I’d share, so other can benefit from my “mistake”.  (Note:  My doctor has given me the green light to make these changes to my dosing without his consultation. Please check with your doctor before changing the timing or dosing of your insulin.)

     

    Typically I give myself two injections of Lantus daily: one at bedtime (usually around 10:00 p.m.) and another with breakfast (between 6:30 and 7:00 a.m.)  One morning, I realized I forgot my morning Lantus dose and ended up giving it to myself at 10:30 a.m.  Later that day, I noticed the Dexcom didn’t show the gradual rise of my blood sugar in the late afternoon to early evening period that I was getting used to seeing.  Maybe the timing of the Lantus dose had something to do with it?

    I decided to test the theory and try giving the second daily dose of Lantus at 11:00 a.m. instead of at breakfast time for several days.  A week later, I’m happy to report, that not only has the timing change helped with my afternoon/evening blood sugars, but it’s also improved my dinner postprandial and bedtime blood sugars! 

     

     I’m not entirely sure why, but I understand that Lantus insulin does have a slight peak to it and there’s some speculation that it doesn’t work effectively for the full 24 hours (hence the decision to split doses in the first place).   So, perhaps, when I gave the two doses of Lantus within 9-10 hours of each other, it left part of the day with less basal insulin coverage… maybe?  I assume that now, with my two doses being spaced closer to 12 hours apart, the basal insulin is working more steadily throughout the day.

     

    Whatever the reason, I’m happy to have mistakenly discovered this improvement to my dosing routine!
