As David mentions, splitting Lantus into two doses is a common recommendation in some circumstances.
However, I'm concerned that you mention 30-40 units of Lantus and only 15 units (or sometimes more) of Novolog. This is not a standard program -- most folks are on a stable unvarying dose of Lantus, and on about the same amount of Lantus and Novolog, rather than a much larger amount of Lantus.
If you are not doing so already, I think you need to see a diabetes team including a diabetes doc, diabetes nurse educator, and a diabetes dietitian. And if you are, discuss my advice with them.
Hope this helps!
Splitting Lantus is a way of evening out BGs a little. But if you're going way high and then way low, you're probably eating too many carbs. Just counting the carbs isn't enough. You need to limit them unless you're titrating you Novolog carefully to the carbs you eat, and it sounds as if you're taking the same amount with every meal.
I think you need more help than we can give you. Please see a doctor or CDE soon and find out how you can improve your diet as well as your insulin regimen.
I'm now off insulin but when I was doing the low-carb (i.e., high meat protein) diet I needed 60 units of Lantus daily. It worked best split 30 in the morning and 30 at night.
Hope this helps.
if your having trouble with your sugar you must eat salads and greens, vegatables, fish, meat, nuts, almonds, fruit in normal servings. what fits in the palm of your hand is what you should be eating for meals. walking is great 3 miles a day or 1 mile butt do it every day not when you feel like it k. remember Diabetis is a Disease not a cure.
You probably need to be a little more agresive with your carb counting. Measure you Blood Sugar 2 hour after every meal, it should be on target. That is the peak time for Novolog. I find that Lantus only last about 20 hour instead of 24. I finally break up my dose in to 2. I do about 18 in the morning and 12 at night, because I go the bed late. You may try the break up the dose depending on the time you sleep.
Maybe you could check out The Bournemouth Diabetes E-Learning Program at www.bdec-e-learning.com It has helped us to work out insulin to carbohydrate ratios, and how to decide your rapid acting insulin dose. Hope this helps

people say weight is the number one problem for Diabetis. not true. sorry. i was a runner for 27 years ate HEALTHY all the time i weighted 178 at 6ft2. when i came down with Diabetis type 2 at the time 13 years later im a type 1 taking 80 units of landis and 140 of Humolog a day. I eat salads, fish, meat, nuts, fruit etc its all in the Geans period. good luck and god bless.
What works for me ( ave 110's in the AM and a 5.8% A1c, which isn;t the greatest but alot better for me)?
1. Goes all the way back to Dinnertime..I have to eat 3 hrs before Bedtime and taking a Bolus of Novalog to be accurate per the # of Carbs, etc.. using carb counting, etc.
2. Novalog last about 3 hrs , it peaks in about 90 min.. and thus My goal after 3 hrs is my Fasting range of 90's
3. At Bedtime? If I am within-in 80-110? I just take my Normal Amt. of Long Lasting ( Used to take Lantus, now take Levemire )
4. IF I Am Above 110? I take a Correction Bolus of Novalog.. Not more Lantus/Levemire..
5. If I am Above 150 at bedtime?
A. I take a Correction Bolus
B. Add 2 more Units of Lantus/Levemire
C. And I set my Alarm for 3 hrs and Test again..
After yrs of Putting Up with Lantus, Getting Alot of overnite Hypo's, after Split dosing it ,etc..? I finally was able to Go to Levemire.. and No More hypo's using it..
Go by your Weight.. ave is a .12 per lb for men.. You weight 200 = 24 units for starters
have you ck'd out trying the 500 Rule?
And Done a Basal Test?
If your eating 30 Tot Carbs and taking 5 units? You have a 1:6 I;CR Ratio..
And that is if?
A. Your starting with a range of 80-100
B. Your 3 hr after meal is the same..
If it Isn't?
Then :
A. If your above 100 B4 eating, have to take a CB ( Correction Bolus)
B. If your Above 140 after 2 hrs? Have to take a CB bolus
C. If your High after 3 hrs? Have to Lower your I;CR to 1:5 next time and keep lowing it until you get in range after 3 hrs..
D. For Figuring a CB? starting with using the 1500/1800 rule has helped me

Many doctors recommend that their patients do split their Lantus dose into two parts. That's because with some people its effectiveness may not last a full 24 hours.
