    I couldn't agree more: the first step to getting the quality care one needs is patient responsiblity and accountability -- in all healthcare situations (including diabetes). People need to speak up, take charge and require this of their healthcare providers. Yet it's easier said than done for most of us; I think that we've been socialized for so long to accept the word of a healthcare provider as gospel and not to speak up.

    Dr. Cogen, I wish the cardiologist I saw last week had 1/2 your smarts. You listen and discuss it would seem wherehe had only met me for a few minutes prior to the Cath procedure, then proceeded to tell me how to control my diabetes, totally closed his hearing of how I control mine, I maintain very good control with diet and oral meds.   He actually wanted me to eat the food provided which I would estimate to contain ≈75gm carbs in one meal.  my intake is 100 to 130gm/day.  My GP and I have worked together and he actually has dropped Actos at my request to see if I would be able to maintain control with just the Janumet, and with the exception of a few days of steroids in the hosp, so far so good.

    Dave: thank you for your kind remarks. I am delighted that you are empowered to know what is in your best interests. Clearly, it seems that you have an excellent therapeutic relationship with your GP who will listen and base advice on your mutual goals. In regard to the cardiologist: he performed the procedure; hopefully without complications and now you can go back and work with the healthcare professional who knows you best. (be prepared for high blood sugars with steriods, though).
