Latinos! A Survey for you

Hi,

 

The Dermatology Department from Stanford University would like to invite you to fill out a questionnaire. This survey is completely anonymous. We are investigating knowledge, attitudes, and practices regarding skin ulcer prevention among Latino adults.

 

Our survey takes approximately 10 minutes to fill out and is available in both English and Spanish. You must be 18 years old or older to fill out this survey, and identify yourself as Latino, Hispanic, Mexican, Central American, South American, Puerto Rican, Cuban or Dominican.
