I would say everything you listed above! plus fast food lack of exercising and genetic's.  I know a lot of people believe people who have Type 2 are overweight, I disagree. I know a lot of Type 2 diabetic's who are normal weight and still develop Type 2. We need to stop eating fast food, learn how to eat healthy and exercise at least 30 minutes a day.  We need to teach our kids how to play outside and keep the JUNK food out of the homes.  In my own opinion, I believe that will help a lot.
