When should I consult an endocrinologist?  I have type 2 diabetes... the previous medication and diet regime was no longer working so my physician has started me on a combination of diet, insulin, metformin, and byetta.  I am still struggling to control the blood glucose spikes... should I go to a specialist at this point?
