Q: Hi: I am a type 2 diabetic and insulin dependent.

My doctor just added fast-acting Novolog last week. She put me on a sliding scale, but I am finding out that it is better to inject according to how many carbs I am eating. Is this the better way, or is the sliding scale better? If my prepranial sugars are under 150 mg, and I don't inject, when I eat, I do spike a lot after about 1-2 hours or so. So I am thinking that insulin to carbs ratio is better. What are your thoughts?

My son (type I for 25 yrs) just switched over to Lantus and Novolog.  He likes using a specific diet combination for his three meals (certain numbers of carbs and fruits etc) and has a specific Novolog dose for each of the meals.  He slides the Novolog dose for each meal based on his BS level at that meal---about 1 unit NL for each 30 units of BS above 150.  A friend of his just looks at his meal, estimates the content, and calculates the NL dose from that.  My son believes he does better with the specific diet--which also yields better weight control.
