In the UK/Europe Januvia is only licensed to be given once daily. This dosage will have been decided upon based on the trial evidence submitted by the company (Merck). As such the twice a day regimen prescribed is not licensed and may not offer any additional benefits over 100mg once a day but increase  the risk of side effects.
