As you may know from the Januvia patient information, Januvia has not been studied in patients with type 1 diabetes or in combination with insulin. This is true, although there is one small study where it was used in combination with insulin in a few patients. Unfortunately, there is no way to make any conclusions about its safety from this information.
Because it has not been studied, the risks of this use are also unknown. There is always a risk of dangerous low blood sugar (hypoglycemia) with insulin, and there is a possibility that Januvia would increase this risk.
If you are concerned, you would want to discuss with your physician their experience using Januvia in this way in other patients. You would also want them to help you fully understand the risks and benefits of this use.
