Where is a sliding scale for insulin available?

Usual a sliding scale for insulin is set-up by your primary care doctor. General rule is for every 50 points (blood sugar) take one unit of insulin (regular). Example, fasting 200 (should be 70-110), take 2 units. After dinner should be 140-180 at the most. So anything over take 1 unit for any amount of those numbers.

My Doctor has me using a sliding scale for Humlin R and has had me doing so for over 4 years now. He has me taking 1 unit for every ten points above 120. So if my blood sugars are say 140 I would take 2 units. But it is best if you gt the scale your doctor wants you to follow from your doctor. Everyone is different.
