 You would be logically switched to Levemir only if your kidney function was going downhill.

  Lantus is the best insulin for those with normal kidney function (eGFR > 60)

  Levemir is best for those with reduced kidney function (roughly eGFR 30-59).

  Those on dialysis should start with a oral medication like Prandin, insulins with

short half lives may be added once or twice per day.
