Q: Could my daughter be diabetic?

My daughter is 5yrs. old.  I have 2 nephews who are T1 Diabetics and my mother-in-law is also T1 diabetic.  So there is a lot of family history. About a year ago I noticed she was going to the bathroom a lot.  Use my nephews test kit and tested her blood sugar it was 259.  Made her fast for 12 hours and it was 137.  She ran high off and on for about 2 weeks then was in normal range again.  In the last month notice her using the bathroom, drinking a lot, irritability.  Complained of stomach pain and even came home from school early one day. Tested her Blood sugar level again and it was 194 before bed.  Fasting till morning she was 117.  Her Blood sugar has been erratic. Almost alway above 105 to 120 in mornings. Suddenly she is back in normal ranges again but teatering on the border of being high.  Had tested urine and not spilling sugar(used restroom 2-3 time before sample).  A1c was 70.  Can anyone help me on whats going on?  Or what to do now?

Good Morning: I am concerned that your daughter may, indeed, have type 1 diabetes in view of the fact that her hb A1c is 7.0 (you wrote 70 but I think you meant 7.0). A hb A1c of 7.0 is a 3 month estimated blood sugar average of 154.2 mg/dl. Some professionals diagnose diabetes by using a hb A1C>6.5. In your daughter's situation, she is having symptoms, elevated blood glucoses via meter and an A1c of 7.0 which is highly suspect. It is very important that you have her seen by a pediatrician with some experience or an endocrinologist in order to do all the labs necessary to diagnose type 1 diabetes. They will need to actually do a laboratory blood sugar as meters are not exact. In view of the family history, it is important to check for the antibodies as well (GAD-65 antibodies etc.). Please follow-up with us to let us know what happens.

Good luck,

Dr. Cogen
