Get An i-port® Now
The i-port® is a prescription only device. If you think that you can benefit from using the i-port®, please print the Prescription Order Form and take it with you when you speak to your healthcare professional.
Once the Prescription Order Form has been completed and signed by you and your healthcare professional, follow the instructions on the form to fax it to the i-port® team. A Customer Service Associate will contact you within 48 to discuss the status of your order.
