First, this is a guy really missed as there are too many forgettable talking heads. Russert was in the elite class in what he did in his job.

However, early photos show the cigarettes, so with CAD, he was still definitively overweight, which means most probably elevated LDL, by other appearances probably had inflammatory disease, and was one of those guys who was unfortunately fat on the outside as well as inside. My guess would also be that he refused other diagnostic coronary testing.
The additional lesson here is, he probably ( although we do not know ) was not following all of his doctors orders and made the fatal mistake of thinking that medication and exercise were enough. With his history, only the COMPLETE program improves the odds. Get the weight off, change the diet, lifestyle modification, in addition to drugs, exercise and maybe testing.
