Heart Healthy (low sodium) and Diabetic diet.

I need both a heart healthy (low sodium) and a diabetic diet. Where can I find it?  I am overweight, have Congestive Heart Failure and am diabetic.  I have been searching the internet to find what I should and should not eat to help with both problems.  My blood glucose level is out of control.  I can't exercise because my heart is too weak.  I get out of breath just walking around my apartment.  My ejection factor is 15%.
