Q: can the first outbreak start in 4 days and last as long as 30 days?

It is not likely to last that long but every person is different. It is hard to say. I would talk to my doctor about the length of time of the outbreak you are having. I would also ask him/her to put you on valtrx to help stop the outbreaks. Valtrex makes a big difference.
