    THE FOLLOWING ARE MY FATHERS REPORTS.

    AGE: 59 (MALE)
    KNOWN HYPERTENSIVE AND DIABETIC FOR THE PAST 10 YEARS ON TREATMENT.

    UNDERWENT CABG ON 1999 FOR COMPLAINTS OF CHEST PAIN WITH ANGINA WITH 3 VESSEL BLOCK.

    UNDERWENT ANGIOPLASTY WITH STENT FOR LAD VESSEL PRIOR TO CABG .

    CABG - 2 VEIN GRAFTS USED , SVG TO D1 AND SVG TO RCA .

    LEFT RADIAL ARTERY GRAFT USED FOR LAD VESSEL

    PATIENT WAS ASYMPTOMATIC SINCE 1999

    ON 2006 HE HAD HYPER TENSION (210/100) AND ADMITTED

    2007 ROUTINE CARDIAC EVALUATION WAS DONE

    (TREAD MILL TEST (TMT) NORMAL WITH 11MIN EXERCISE TOLERANCE

    2D ECHO NORMAL STUDY

    HOLTER - NORMAL STUDY)

    CT ANGIOGRAM STUDY (29/MARCH/2007) SHOWED PATENT RADIAL ARTERY - LAD GRAFT DIFFUSED DISEASE SEEN AT LEFT CIRCUMFLEX ARTERY , COMPLETELY OCCLUDED BOTH VEIN GRAFTS (SVG - D1, SVG - RCA)
    RCA AND AM BRANCH SHOW NORMAL CALIBER OF LUMEN
    PSEUDO ANEURYSM SEEN AT ASCENDING AORTA 3 CM ABOVE THE AORTIC VALVE , SIZE OF THE ANEURYSM 3.7 * 2 CM , CROSS SECTIONAL DIAMETER OF THE AORTA AT THE LEVEL OF ANEURYSM - 5.3 * 3.7 CM
    PSEUDO ANEURYSM WAS FOUND INCIDENTAL PATIENT HAD NO SYMPTOMS .

    TO CONFIRM DIAGNOSIS TRANS ESOPHAGEAL ECHO CARDIOGRAM WAS DONE ON 30/MARCH/2007  WITH FINDINGS OF SMALL ANTERIOR AORTIC ANEURYSM MEASURING 2.7 * 2 CM,(AORTA MEASURES 5.1 CM AT THE SITE OF ANEURYSM). MARKED ATHEROMA IN ARCH GRADE- IV NO MOBILE ELEMENTS .GRADE III ATHEROMA IN DESCENDING AORTA. NORMAL LV SIZE AND FUNCTION. NO INTRA CARDIAC THROMBI .

    PRESENTLY PATIENT IS ASYMPTOMATIC WITH CONTROLLED LEVELS OF BLOOD PRESSURE AND DIABETES ON TREATMENT.

    OPINION NEEDED REGARDING THE INCIDENTAL FINDINGS OF THE PSEUDO ANEURYSM OF ASCENDING AORTA AND GRADE - IV ATHEROMA IN ARCH OF AORTA.AS THE PATIENT IS ASYMPTOMATIC.
