could this be high blood pressure bec. of salt/sodium content? some other warning?

symptoms of 48 year old man; lately when eats sunflower seeds feels all pumped and easily gets angry...other health isssues: takes testosterone shots for low test./night sweats/ 5'8" 240lbs/ownes bus. very high stress/no exercise/perfectionist/drinks at times 1-4 drinks, smokes occasionally, sleep apnea/hi triglycerides. family history (Father): heartattacks (?2, still kickin at 82 yrs old), heart surgeries, diabetes (1-4 shots insuliln a day),angina, congestive heart failure, mom: diagnosed with dementcia recently (85 yrs old)... what should this man i care about do? how urgent/serious are all these factors put together? please help me help him!
