How concerned should I be about a 50% blockage in 1 artery and a 40-45% in 2 others?

I recently had a heart cath done 5 years after receiving a stent for 90% blocked left anterior descending artery.  I am trying to decide how drastically to change my lifestyle (job, diet, etc...)

This question would be best answered by a doctor who could see you in person. With out knowing you or your lifestyle it is difficult to judge how concerned you should be.
