My ejection fraction is 15%, when is a person eligible for a heart transplant?

I was diagnosed with CHF in March 2008. In July they discovered I have hyerthyroidism. My EF is still at 15%. They will soon make a decision on a defibrillator. At what point does a doctor consider a heart transplant?
