my chest hurt really bad mostly left and sometimes right side

i have been getting really bad chest pains mostly on the right side and sometimes on the left side. it kinda helps if i push really hard on my chest for my pain to start to go away. I check my BP when it hurts and its around 149/94 when it hurts bad. what could be going on?? heart attack, blockage, hypertension? i am 31 yours old male white smoker 6'1" 190 pounds.
