drug interactions, pharmacist review, LA area

Is there any type of service for outpatients to have all meds reviewed by a pharmacist for interactions?  My 83 year old Father has a long history of CAD and DM type 2.  He is seen regularly by his cardiologist and endocrinologist, and they both write prescriptions.  It seems they do not communicate often, and my Father would greatly benefit by someone who would be willing to review his meds, as a client, also considering all of his experiences on the drug regime.
