I have nausea, pain in my chest that gravitates, shortness of breath, burning/itching on my chest

My nausea has been for several weeks.  My shortness of breath occurs usually when I bend over ot pick something up.  I also have some pain under my left shoulder but it is short lived.  My father had a heart attack in his 40's - I am in this age bracket now too.
