Smoking does not directly cause heart enlargement, but its effects can lead to heart enlargement.  Smoking is associated with an increase risk of coronary heart disease and heart attack, high blood pressure, and an increased tendency for the blood to clot.  These issues can all lead to a weakening of the heart muscle and an enlargement of the heart.
