Without being able to see skin condition you describe, it is impossible to tell what is wrong.
That being said, one of the more common complications of ICD implantation is infection. An infection of the ICD pocket/incision would show the skin around the site to be warmer, more red, painful and swollen than the non-affected side. Also, you might notice a fever, chills, shaking/rigors, and other "flu-like" symptoms.
Regardless, you should make an appointment with your doctor, if you do not have one soon, to discuss if your symptoms/skin condition is an infection or not.
