I'm not the best source for guidance when it comes to medications. It's best to speak with a physician or pharmacist for instructions on your meds. Let me give you some input on the vitamins and mineral.
Vitamin D:
It's necessary to take calcium when you supplement vitamin D, so find a supplement that includes both vitamin D and calcium. A vitamin D level of 11 or less does indicate you're deficient of vitamin D and should supplement. The current recommended dose of vitamin D is between 200-400 IU depending on age; however, there is discussion on raising this level based on recent research. Some are urging an increase to 2000 IU, so very significant increase. Right now, you do not want to supplement over 1000 IU per day without consulting your physician. Due to your low vitamin D lab results, I encourage you to consult your physician to determine the right level for you.
Vitamin K:
You need between 100-500 mcg daily. I have looked into your medications, but I'm not comfortable giving you a recommendation. It's best to speak with your MD. Your meds do affect blood viscosity, so medication dosage may need to be altered depending on vitamin K intake. Speak with your physician.
Chromium:
Chromium is linked to improved blood glucose control by improving the effectiveness of insulin. It's recommended you receive 50-200 mcg daily, so your intake of 165 mcg daily falls right within this range. FYI - Double check that you are supplementing mcg, not mg.
