I'm hoping you have a pulse oximeter at home. If not, he should have his oxygen level checked as soon as possible.
Make sure you insist that they look for low oxygen levels with activity. ANY reading below 92% IS something to be concerned about, though insurance companies usually won't pay for home oxygen until you reach 88%.
Does he wake at night gasping for air? Does he like to sleep propped up?
How about swelling at the ankles?
