My husband has "roller coaster" blood pressure. What are the possible causes and treatments?

My husband has had diabetes for over 15 years, and end-stage renal failure, (on dialysis) for three years.  Starting several years prior to the renal failure, and continuing now, he has had frequent, sudden spikes in his blood pressure (such as 220/140.)  Other times, his blood pressure is excellent, and sometimes it is too low (such as 80/50.)  He has been on blood pressure medication for many years, but nothing seems to stop the peaks and valleys, and no pattern has been discerned. Can you tell me of any possible causes, and any treatments?
