b/p spikes 230/110 and severe headache and pain over the ear.

Hi I have been on blood pressure meds for several years. Lately I am having middle of the night b/p going as high as 230/110 with excruciating headache. Dr. has done 24 hour urine ultrasound blood work echo and stress test , all came out good. has upped my bennicar to 40 mg daily with 5 mg citalipram and bystolic 5mg at bedtime and clonidine 0.1mg tab hcl as needed for spikes. last 3 to 4 days have a sharp pain that comes and goes with my heart beat located above my left ear . I am 62 female very active and have been diagnosed as having reflux for persistant cough feel like I am falling apart. Any ideas?
