Q: 81 year old Grandma has uncontrolled hypertension.

She currently uses:

 

Exforge 5/320 mg - once daily

Isosorbide Mononitrate 120 mg (ER) - once daily

Carvedilol 12.5 mg - twice daily

Furosemide 20 mg - once daily

Klor-con 8 meq sustain release tabs - 2 daily

Clonidine 0.2 mg - 1 daily

 

Her BP can be as high as 190/ and as low as 90/. 

 

Are there any other medications which may stablize her BP.  Kidneys, heart check okay so far.  She has had low sodium levels twice in the past year.

Your pharmacist may be a good resource to discuss the pros/cons of different medications that you could then discuss with her physician.
