Q: Frustrated Patient - Is it IBD, IBS or possibly something else? HELP!

In 2005 I was hospitalized and diagnosed with Infectious colitis and a caecal ulcer in my terminal ileum caused by Campylocater jejuni. When symptoms were still present 5 months later (eg Abdo pain, C & D, intermittent bleeding, High WBC) I was prescribed pred (after stool tests cleared any infection still present )and told by the GI that they were sure I had IBD. So after 5 months of being on pred and symptoms had eased by the drug I was weaned off and underwent another colonoscopy. This time it was normal. I had a small bowel barium x ray done a few months later and it also showed as being normal. Now 4yrs later and after many GI appointments I have had another colonoscopy yesterday that looks all clear. Biopsies were taken and sent off for testing and I'm currently awaiting the results. As you can imagine I'm completely at a loss and am desperate for an answer. I feel like I'm going crazy. My recent WBC is still high and I have D/C, fevers, joint aches, lethargy, nausea and bleeding. My questions are: Can a colonoscopy show as being normal but a biopsy result show otherwise. Is it possible to have normal results for both the colon and barium x ray and still have IBD? What are the chances that it's IBS? How long does it generally take to be properly diagnosed? My next appointment is in 3 weeks and I'm desperate for some other opinions. I'm unsure if this information helps - my grandfather was diagnosed with Chron's Disease. Thanks....

Hi Becca,

Welcome to the site and I'm sorry to hear that you're still having problems.

It is possible for a scope to look okay and then for a biopsy result to show something called microscopic colitis (MC). "Microscopic" because the inflammation in the cells can be seen only with a microscope. With IBS - Irritable Bowel Syndrome - symptoms like fever and joint aches are not present. Even though it's hard to wait for the results and your appt. that 's what you need to do. If it is MC the treatments are the same as those used for Ulcerative colitis. However, all that said, constipation is not a typical symptom of IBD, so there could be another reason for your symtoms as well. Diagnosing gastrointestinal issues is not easy nor as cut-and-dry as we would like. It can take time and patience.

But, in the meantime go back and read some of my Shareposts about diet and keeping a food journal and get started doing that to see if anything that you are eating or drinking is making your symptoms worse or better. Then take in your journal when you next see your GI.

Above all, try to remain calm - hard, I know, but the gut is the root of all our emotions and getting frustrated, angry, or scared can only make symptoms worse.

Good luck,

Elizabeth
