I would recommend you see a physical therapist or a chiropractor in your area.  With a direct fall on your front and back side, as you have described, you may have suffered an injury to your coccyx (tail bone) and/or your SI joint.  Both of these injuries can lead to inflammations that can compress on your perineal nerve and effect bladder control.  Fixing your alignment and biomechanics may be a great step toward resolving your pain and incontinence issues.
