Q: what is causing my constant urge to urinate following every urination?

You may not be fully emptying your bladder, so you are getting signals from your bladder to your brain to urinate.  First, try to practice "double voiding."  That means stopping, sitting on the toilet again relaxed to try to empty the bladder.  You may need to visit a doctor to investigate residual urine in the bladder to determine if retention is a problem.  The doctor may prescribe urecholine to help the bladder contract more effectively or suggest you use self-catheterization to empty your bladder completely, depending on what is found from the diagnostic tests.
