How important is changing right before being seen by the Doctor?

Hi, Everytime I have to see the Doctor I always have issues if I have leaked and my diaper is damp. I change right before I leave for my appointment, but as usual I always leak. I do appologize, and she acts like it is nothing, but I do wonder if I should change in the restroom at the...
