Q: incomplete quad with intact sensation has pain in penis when catheter bag is moved to reposition

the incomplete quadreplegic that I care for has a suprapubic cathether, he has sensation that is why he got the suprapub because he hurt alotwhen being cathed with foley plus he kept getting infections. but now when you move the catheter bag to reposition him like putting him to bed he says it hurts it feels like he is being cathed through his penis when you move the bag. what causes that pain and how can I help him? he always spasms really bad for a week or so after it first gets changed and he leaks, the urologist gave him a transdural patch for spasms and it cuts back on them some but not alot. after a week or so he stops leaking.
