Q: Why do people with dementia and altheimers get UTI's and bladder infections almost constantly?

My mother is in a nursing home with dementia/altheimers, she has inconstinence along with a colosomy from rectal cancer in 1985.  She has been in the nursing home since

2/09 and she has constantly had either a UTI, bladder infection, or most recently a severe yeast infection that was so bad it had gone to her bladder.

She had been with a catheter on and off for a period of about 2 wks all together and given antibiotics intravenously. 

She kept complaining with severe pain and a feeling of a lot of pressure in her bottom in.

When we questioned the nursing staff, they said she was just having bladder spasms, yet she complained constanly of feeling raw in her vagina.

After taking her to an urologists he said she had a very, very severe case of yeast infection.  He said no more catheters! He put her a different medication and said she should be feeling alot better.

What do we need to do to keep this from happening again?

A few thoughts:  The use of antibiotics to treat UTIs makes one more susceptible to yeast infections.  Dietary supplementation of yogurt with active cultures may be of benefit.  Meticulous perineal hygiene, wiping front to back may need to be coached.  Is she being toileted?  In a lot of older folks, bladder tone is less than adequate which leads to residual urine in the bladder.  This is a common reason for the recurrance of infections.  Toileting at intervals will help especially if her product is changed as needed at these times.

I work in a nursing home as a nurse aide and my experience is they do not drink enough du to confusion.  also with incontinence being an issue with bowel and bladder cleanliness is a culprit as well.  This is my experience.

I thank you for your reply.  The problem is she does drink alot, we make sure

she has water, cranberry juice and she love grape flavored water.  She is also

very consciensous about her colosomy bag and cleans it very well.  When she

urinates she uses wet wipes to clean herself and the dry wipes with toilet tissue.

We do have a problem making sure she washes her hands with antibacterial soap.

The nursing home she is very slack about keeping plastic gloves for her or any one

that is in there to assist her.  They also are very slack about disposing of her soiled

diapers and pullups. 

Any suggestions.  Thank you for your help.

I work in a nursing home as a nurse aide and my experience is they do not drink enough du to confusion.  also with incontinence being an issue with bowel and bladder cleanliness is a culprit as well.  This is my experience.  Also over use of antibiotics as with anyone reduces how they will heal the UTI's  Catheter removals also make them feel as though they gotta go as well i gives me an urge when my urologist caths me. I am 36 yrs old with bladder issues and work in a nursing home and since I have had bladder issues it has opened my eyes to how they must feel.

Very simple.....nurses are lazy, they donot keep the areas clean every day several times a day.....tubes are not replaced every day......you have to stay with her, watch her being cared for and get down these peoples throats.....every day till they do their job. They donot care........period.
